Explore and pilot a neighborhood and community-based Care Response Team (PSW2, Medical Case Manager, Community Health Worker) that respond to 5150/5585 linkages that did not result in a hold to provide a warm hand-off to services and continuity of care. (Address needs of high utilizers).3. School district specialized staff are trained in risk assessment and have protocols in place to provide care and transitions for students.4.
